Cayla is contracted with UCLA Medical Group HMO Insurance (must have approved authorization).

Cayla Kurland Speech Pathology is also an out-of-network provider for PPO insurance companies. We are happy to provide you with the appropriate documentation to submit for reimbursement from your insurance company.

 

We cannot imply or guarantee any specific benefits your PPO insurance company may or may not provide.

Payment (Out Of Network)

Payment is due at the time of service. Payment is accepted in the form of cash, check, and all major credit and debit cards.

Prior to receiving speech therapy services, we advise you to verify benefits with your insurance provider. Recommended questions:

 

  • Does my policy provide for speech therapy benefits?

  • Is pre-approval required? And, if so, what is required for pre-approval?

  • What is the reimbursement rate?

  • What diagnosis codes are acceptable for speech therapy reimbursement?

  • Do my benefits cover speech therapy as well as the evaluation?
